In a new partnership with the Alzheimer Society, the Dementia, Resource, Education, Advocacy, Mentorship (DREAM) team program has been implemented in Brantford General and Norfolk General hospitals with the goal to assist dementia patients and their caregivers with early intervention, information and assistance in order to keep dementia patients out of emergency rooms and avoid lengthy hospital stays.
