An Alzheimer’s Society of Ontario pilot project aims to provide families with a helping hand when it comes to the transitional pains and stress that can be felt when loved ones move from being cared for at home to living in a long-term care facility with the aid of an assigned healthcare worker that will guide the family through the transition and up to three weeks afterward.
